Services include Home Care Services (skilled nursing; licensed nursing assistants; rehabilitative therapies; medical social workers; telehealth monitoring and medication scheduling), Specialty and Family Services (certified wound/ostomy nurses; certified psychiatric nurses; certified lactation consultant, diabetic educator; certified intravenous nurse; newborn-post partum/pediatric nurses), Support Services Programs (personal care service providers; homemakers; staff companions), Adult Health Screening Clinics (blood pressure, blood sugars checks, cholesterol screenings, foot care, health vaccinations, medication questions answered).
