The ServiceLink Resource Centers network consists of thirteen community-based ServiceLink Resource Centers and many satellite offices with the common purpose of providing information and supportive referrals about resources for older adults, adults living with disabilities, chronic illness, and their families and caregivers. ServiceLink Resource Centers are trusted places in your community to get answers about healthcare and other services for older adults, people with disabilities and caregivers. The ServiceLink Resource Center team is one phone call away. We'll listen to your needs, respect your privacy and help you find answers.

Our Services:
  • Senior Health Insurance Program/Senior Medicare Patrol (SHIP/SMP)
  • Connecting Medicare Beneficiaries to free and confidential health insurance counseling services that may include support around Prescription Drug Plan enrollment, Fraud and Abuse, Medicare Savings programs and Low Income Subsidy (LIS) application assistance. SHIP also provides a variety of Medicare publications, reading materials and training opportunities to become a Medicare volunteer in your community.

  • Transitions in Caregiving
  • A Caregiver is anyone who provides regular care for a spouse, relative, a partner or a friend. The Transitions in Caregiving program provides education, training, supportive referral and information services to local resources, support groups and other services that may support the caregiver.

  • Information & Referral
  • Information and Referral services provides general assistance to consumers looking for resources regarding, but is not limited to, subsidized housing, medical assistance, fuel and electric assistance programs, commodity foods, food pantries, emergency medical response systems, legal advice and other requested services that may be available within the community.

  • Long-Term Care Support
  • Long Term Care information and counseling referrals are available to caregivers and persons being discharged from rehabilitation sites, hospitals or those seeking care plan development. Long term care planning offers supports to those seeking options that meet their health and personal care needs over time.

  • Division of Family Assistance
  • The Division of Family Assistance conducts financial eligibility determinations and processing for individuals who are applying for Medicaid, Choices For Independence (CFI), Long Term Care Medicaid, Medicare Savings Programs or Supplemental Nutrition Assistance programs (SNAP/Food Stamps).
